Support at Home Program Services
Support at Home Program Packages provide individualised coordinated care and services for seniors with basic to complex to intensive ageing related care needs.
Our team of dedicated care partners, support workers and brokerage partners work together to assist you to get the most value out of your package so you can continue living well, safely and independently at home.
PAWA Community Care is an approved provider of Australian Government-funded Support at Home services.

CHSP Program In-Home Support Services
Our carers provide entry-level support to help you live independently at home. We provide basic care and services to those who require minimal assistance with essential tasks that they may find challenging as they age.
PAWA Community Care is an approved provider of services funded by the Commonwealth Home Support Programme (CHSP).
Under CHSP you can access a range of services suited to your needs. CHSP is for seniors needing 1 or 2 entry-level services.
Low-level needs on an ongoing basis
Short-term support after injury or illness
Access to basic, entry-level support services with minimal costs
Interim support while waiting for a Support at Home care plan.
Flexible Respite for Carers
Our flexible respite services give family members and carers time off, to rest and recharge while we look after your loved ones.

Service Program Models
We provide services through two program models – avaliable through Government funding.
The Australian Government subsidies a range of services that can support people to live longer, actively and safety in their own home. Depending on your care needs and circumstances, our services can be accessed through the Commonwealth Home Support Program or Support at Home Program.
1 - Commonwealth Home Support Program (CHSP)
Designed for those looking for short-term/casual help or basic assistance around the home, the Commonwealth Home Support Program lets you choose how to spend your money, when you need it. You’re not locked into anything. CHSP is for people who only require 1 or 2 entry-level services at home.
At PAWA Community Care, CHSP funds domestic assistance, transport, meals, personal care, social support, nursing and allied health across specific funded areas in NSW.
Our CHSP services currently operate in South East, South West, Inner West & Western Sydney Regions of NSW.
CHSP is suitable for those who require:
Entry Level Care
Low-level needs on an ongoing basis
Short-term support after injury or illness
Access to basic, entry-level support services with minimal costs
Interim support while waiting for a Home Care Package
2 - Support at Home Program
For long-term, ongoing support, PAWA Community Care provides services through the Support at Home program. There are eight funding classifications available (Classification 1–8), with funding allocated based on an individual assessment of your care needs.
If you require multiple services then a personalised Support at Home care plan is the best option for you. Care plans are designed to be tailored, flexible and adaptable for those with more complex care needs that go beyond what the Commonwealth Home Support Programme can provide such as multiple ongoing services or access to specialised services such dementia care. You can work with our team to design a care plan to choose services and decide what support you want to receive.
Our Support at Home services currently operate in the whole Metropolitan Sydney.
Support at Home is suitable for those who require:

New to Aged Care?
We’re here to help you through the process and get you started on your home care journey. With an understanding of the process inside out, our friendly team can help you navigate all the services and government funding available, so that you can make informed choices about what is best for you and your family, including free assistance with your My Aged Care assessment.
An assessment process is required as it determines what program is suitable for you or your loved ones. The Australian Government subsidises a range of aged care services - depending on your circumstances, care needs and assessment outcome, our services can be accessed through the Commonwealth Home Support Program (CHSP) or the Support At Home Program - which has replaced the Home Care Package (HCP).
You may be eligible for subsidised funding. There are 2 types of funding models available to access our Aged Care Services:
All applications for government-funded aged care services are made through the government portal, My Aged Care.
If you are finding the application process confusing or difficult please call us directly 1300 227 292 to speak to a care expert at PAWA or book a consultation and we will happily assist you through the process.
Already Assessed?
If you have already been assessed and are looking for a provider contact us directly to enquire about our services and what we can offer you. Provide us with your referral code and we’ll discuss your circumstances, needs and goals, services available in your area, your culture and language options, our staff and our cost and fees.
If you want PAWA Community Care to be your provider of choice. You will then enter into an Agreement with us and together we will create an agreed care plan. Once you are happy with the arrangement, your services can begin.
Looking to Switch Providers?
If you are already receiving assistance from another provider and are thinking of changing provider, call us to discuss what we can offer and design a better plan together.
Call us on 1300 227 292 and our team will organise a meeting. We want to get to know you, your previous experiences and what you need, to ensure we cater your services especially for you.
Switching your package provider to PAWA Community Care is easy – we’re here to make the transition as manageable as possible .Get in touch with our team members, we’ll give you a referral code, you can then inform your current provider to cease services, and then organise a start date with PAWA.

Service Fees & Funding
Home care service fees will depend upon the services you need and the funding allocated to you.
At PAWA Community Care, our primary focus is to understand the support you need, the services you want, and what you can afford. We will work with you to determine how this can be best structured within the aged care environment.
We can assist you to access Australian Government-funded aged care programs based on your care needs and preferences. Depending on your circumstances, you may be required to contribute towards the cost of some services under the Support at Home program. Any contribution is determined in accordance with Australian Government funding arrangements. Our coordinators can explain your funding options and help tailor the most suitable program to meet your needs and budget.
CHSP Funding
CHSP is part-funded by the Australian Government. If you can afford to, you may be asked to pay a contribution towards the cost of your CHSP services. This contribution is paid directly to your chosen provider, such as PAWA Community Care, for the services you receive.
Support at Home Funding
Support at Home funding is allocated based on an aged care assessment and your individual care needs. You will be assigned a funding classification (Classification 1–8), which determines the level of funding available for your approved services.
Your Support at Home funding is not paid directly to you. Instead, you choose an approved provider, such as PAWA Community Care, to coordinate your services, manage your funding, and ensure you receive the support you need to continue living safely and independently at home.

Our Cost of Service Fees
Our fees are competitive, open and transparent to ensure we provide the best care.
When setting our fees, we consider the costs of wages, travel time, mileage, training and development, and administration to deliver safe and reliable services.
Depending on the service, our Support at Home and CHSP fees are based on an hourly rate, per visit, or per service.
Under the Support at Home program, service charges are deducted from your individual funding budget. The hours and types of care you receive will depend on your funding classification, approved services, and any applicable participant contribution.
Under the CHSP program, the cost of services depends on the agreed level of support and the hours of care delivered within the scope of services you have been assessed to receive.
You remain in control of your care and how your funding is used, giving you choice and flexibility in the services you receive and the way they are delivered.
